ABOUT REGISTARTION & UHID 
To avail any service at Apollo Hospitals Dhaka, all patients are required to register first. This once-only registration is simple and easy. Once registered, you will be given a UHID (Unique Hospital Identification) number and card. This UHID number (your unique patient number) will serve you lifelong and through this, the hospital will create an accessible online database that will store all your medical records and prescription while you are being treated under Apollo Hospitals Dhaka.
HOW TO REGISTER
To Register:
- Collect a Registration Form from "Information Desk" at the hospital's atrium or download Registration Form and print it
- Fill up the form with your correct personal information, contact details and signature.
- Submit the Registration Form and registration fee at OP Billing counter (also located in atrium).
- Our officers at the billing counter will process your registration and provide you your personal UHID card having a unique patient identification number.
FREE UHID FOR NEWBORNS
Babies born in Apollo Hospitals Dhaka receive UHID numbers without any charge. Temporary registration is first provided, which is later replaced by permanent registration. The hospital also provides hardcopies of the registration to parents of the newborn.
